Tips for a successful appointment:

  • Please arrive 15 min before your scheduled appointment.
  • Bring your insurance card and valid Picture ID.
  • Bring list of medications your are taking.
  • If your insurance requires a referral please bring with you.
  • If you have been seen by another ophthalmologist please bring your records with you.
